But how on earth do you add an existing file ?

I want to include /usr/X11R6/include/X11/XLib.h

but I can't see how to do this anywhere (although creating and adding new user files is easy). How do I do it ?

Open the automake manager by clicking on the lower icon on the left side, it has a piece of paper and screwdriver icon. Select the subproject folder at the top, and right click the target at the bottom, add existing files. It's in the Handbook documentation.

You probably just want to use an #include statement, but have that path in your include paths. I haven't figured that out yet.

That was tricky, no wonder you didn't find it.
